Each individual has different support needs so, the HACC program has developed a range of services to meet a variety of needs of frail aged people, people with disabilities and their carers.
HACC services must carry out an assessment of a person's needs in order to provide a service. A worker from the service may make a home visit to carry out an assessment and ask questions that will help them to determine the range and level of assistance required.
The assessment may lead to an agreement with the service provider about what service will be provided and any service costs as outlined at What will it cost?
Provision of these services may also depend on availability of services as outlined at Are you eligible?
As a person's needs change, HACC services will adjust what is provided.
