People needing a HACC service should:
- Contact the service directly
or
- Have a friend or relative make contact
- Have their doctor refer them to a HACC service
- Have another service provider make a referral
The person needing a service will then have an assessment by the HACC service provider.
Contact a service by either:
Phoning any of the services listed under Service types.
or
Phoning the Commonwealth Carelink Centreon 1800 052 222 for details of any HACC service.
or
Contacting one of the assessment or case management services listed under Case management to organise a range of services based on your needs.
